Adults Transitioning From Incarceration: Reentry

LG Links Inc. provides whole-person reentry support for adults transitioning from incarceration. Through pre-release planning, care coordination, and warm handoffs to community providers, we help members connect to primary care, Medication-Assisted Treatment (MAT), behavioral health services, housing, employment resources, and essential daily supports that promote a safer, healthier return to community life.

Youth Transitioning From Incarceration: Reentry

LG Links Inc. supports youth transitioning from juvenile justice involvement and other systems of care through wraparound, trauma-informed services. We help young people access healthcare, counseling, recovery support, stable housing, education, job training, and personalized transition planning so they can move forward with confidence, connection, and long-term stability.

Adult/Youth Birth Equity

LG Links Inc. is committed to advancing birth equity through culturally responsive, trauma-informed care for pregnant and parenting individuals from underserved and justice-involved communities. By partnering with doulas, midwives, healthcare providers, and community programs, we help families access prenatal, birth, and postpartum support, reduce barriers to care, and strengthen maternal and infant well-being.

Adults/Youth with Serious Mental Illness (SMI) and/or Substance Use Disorder

LG Links Inc. supports adults and youth living with serious mental illness and substance use challenges through compassionate, person-centered care coordination. Our team connects members to behavioral health treatment, recovery services, housing support, peer resources, and community-based care that reduce barriers, promote stability, and support long-term wellness, recovery, and independence.

Adult/Youth Homelessness & Families

LG Links Inc. supports individuals and families experiencing homelessness through compassionate, trauma-informed care and strong community coordination. Our team helps connect members to emergency shelter, transitional housing, food assistance, healthcare, behavioral health services, and other essential resources that address both immediate crises and long-term stability.

Seniors

LG Links Inc. is dedicated to supporting seniors through coordinated care that addresses both health and social needs. We help older adults connect to medical care, housing support, transportation, food assistance, mental health services, and community-based resources that protect independence, dignity, safety, and overall quality of life.

Whole Child Model (WCM)

LG Links Inc. supports children and youth enrolled in California Childrenโ€™s Services and the Whole Child Model through coordinated, family-centered care. This approach integrates CCS-covered services into Medi-Cal managed care and helps children access primary, specialty, and behavioral health services across providers and settings while honoring each childโ€™s medical, emotional, and developmental needs.

LGBTQIA+ (lesbian, gay, bisexual, transgender, queer or questioning, intersex, asexual, and other identities)

We help reduce barriers to healthcare, education, social services, and community support by providing respectful care coordination, advocacy, and trusted connections that strengthen belonging, safety, and long-term well-being.

LG Links Inc. serves BIPOC children, youth, and adults through culturally responsive, community-centered support that strengthens families and promotes mental health and wellness. Our services help members stay connected to care, complete basic health screenings, build healthy habits, strengthen family relationships, and access crisis support when needed for safety and stability.

SNF to Community

LG Links Inc. supports smooth, safe transitions from skilled nursing facilities back into the community. Through enhanced care coordination, discharge planning, follow-up support, and connections to housing, primary care, behavioral health services, and community-based resources, we help individuals reduce readmission risk and regain independence in a stable environment.

Crisis Support

Crisis situations can happen unexpectedly, and having immediate support is crucial. ECM provides 24/7 access to trained professionals who can guide you through emergencies, whether they involve mental health, safety, or urgent personal needs. Our goal is to ensure you feel supported, safe, and connected to the right resources when you need help most.

SMI / SUD (Serious Mental Illness / Substance Use Disorder)

Members experiencing serious mental health conditions or substance use challenges require specialized, compassionate support. Our ECM services ensure access to behavioral health treatment, counseling, recovery programs, and community resources, helping individuals maintain health and independence.

Justice Involved (Pre Release & Reentry)

Individuals who are justice-involved face unique challenges when reintegrating into the community. Our ECM programs provide pre-release planning, reentry support, connection to probation/parole, and access to resources that promote successful transition and long-term stability.

Pregnant & Postpartum

Pregnant and postpartum individuals need comprehensive support for maternal and infant health, mental wellness, and social services. Our ECM team coordinates care with healthcare providers, community programs, and family support services to ensure safe, healthy outcomes.
