Our Justice-Involved ECM program supports individuals transitioning from correctional facilities back into the community. We provide pre-release planning, personalized care coordination, and connections to healthcare, housing, employment, and social support services. By working closely with probation and parole officers and linking participants to a comprehensive reentry resource network, we help reduce barriers, improve stability, and support successful community reintegration.

Eligibility and Referral Pathways for Reentry Support Services

This program serves individuals who are preparing for release or who have recently returned to the community from correctional facilities. Referrals may come from probation officers, parole agents, healthcare providers, community partners, or directly from the individuals themselves. Eligibility is determined by level of need, identified risk factors, and readiness to participate in care management services.
